The Commander, Seventh Fleet, asked CNA to assess the security environment of the Asia-Pacific Region (APR) to the year 2010. In addition to an examination of the major countries of the region, of security trends in Asia (e.g., demographics and weapon development), and of future economic trends in the APR, this assessment warranted an evaluation of Chinese naval capabilities over the period of interest. This research memorandum presents the results of that evaluation. Much of the debate over China's future naval capabilities focuses on whether China will soon have a 'bluewater' navy. In this analysis, we argue that one of China's strategic objectives is to develop a regional navy. We define 'regional' or 'greenwater' navy as a navy capable of effectively achieving China's current regional aspirations (e.g., blockade of Taiwan, seizure of one or more islands in the Spratlys, sustainment of a naval force in the South China Sea, and the ability to inflict damage upon an intervening foreign navy).

This research memorandum is part of a study sponsored by the Commander, Seventh Fleet, to assess the security environment of the Asia-Pacific Region (APR) between now and 2010. It focuses on the most probable evolutionary trends for China during this period. China's emergence as a major regional power will be one of the principal factors affecting the security, politics, and economies of Asia and the Pacific between now and 2010. The forces shaping China's emergence are primarily internal, but include such important external factors as Beijing s perceptions of the intentions of its neighbors and of the United States. Much of the uncertainty about China's future course and impact on the region center on whether, and how, China accepts the norms of the international systems that have grown since World War II - norms that have not yet been tested by the rapid rise in national power of a large non-Western country. Alternative scenarios emerging from the rapid changes underway in China could have widely varying implications for this and other issues.

This research memorandum is part of a study sponsored by the Commander, Seventh Fleet, to assess the security environment of the Asia-Pacific Region (APR) between now and 2010. The study attempts to identify the most probable evolutionary trends in the APR out to 2010, and to derive implications for U.S. forces, particularly the Navy. This memorandum identifies these trends for Japan. The implications for the forces and for the Navy are contained in the final report for this project. The analysis strongly suggests the importance of a stable, credible U.S. presence in Japan in reinforcing Japan s current, yet potentially problematic, pro-U.S. orientation. The U.S. presence will significantly influence Japan' s defense policies because it will determine the credibility of broad U.S. nuclear and other security guarantees to Japan. Such a presence cannot be taken for granted 15 years hence.

The Commander, U.S. Seventh Fleet, asked CNA to assess the security environment of the Asia-Pacific Region between now and 2010. This memorandum identifies the most probable evolutionary trends for the Indian Ocean Region, with particular emphasis on the largest factor, India. Research team members conducted interviews with officials, officers, and scholars in Washington, New Delhi, and Islamabad, and in Hawaii with CINCPAC staff members, and East-West Center and other University of Hawaii scholars. The project team also drew heavily on expert opinion available in Washington, at the Departments of State and Defense, at the National Defense University and from the intelligence and scholarly communities.

As medical care in the civilian sector is moving toward managed care, so it is in the military. The Office of the Secretary of Defense (Health Affairs) has sponsored a series of demonstration projects - limited implementations of health care management programs. One such program is TriCare, in the Tidewater area of Virginia. The project, which began in late 1992, was to have two major changes in the financing and delivery of health care. The program included three options for beneficiaries: Prime (an HMO), Extra (a preferred provider organization) and Standard (the standard CHAMPUS) option. The purpose of this research memorandum is to present the findings from our analysis of changes in the levels of access and satisfaction with military-sponsored medical care. The analysis is based on data we collected for an evaluation of the TriCare demonstrations project in Tidewater, Virginia.

In October of 1992, the Tidewater area of Virginia was designated as a demonstration site for Tricare. The demonstration project makes fundamental changes in the financing and delivery of health care to military beneficiaries currently served by Naval Hospital, Portsmouth; McDonald Army Hospital, Ft. Eustis; and 1st Medical Group (TAC), Langley Air Force Base. Tricare Tidewater is a triservice managed-care initiative, designed to enhance military beneficiaries' access to care, improve mechanisms for quality assurance, control rising costs, and increase coordination between military and civilian components of the Military Health Services Systems (MHSS). Although increasing physician satisfaction was not a direct goal of the program, many of the changes implemented may affect the way that military physicians practice medicine in the Tidewater region and their attitudes regarding their role in the MHSS. Certainly, any positive effects would be welcomed, but a decline in physician satisfaction could lead to lower retention rates for military physicians, as well as lower levels of physician performance, patient satisfaction, and quality of care. In this research memorandum, we measure the impact of the first two years of the Tricare program on physician satisfaction.

This research memorandum focuses on the most probable trends relating to Southeast Asia and Australia during this period. It discusses a few countries and issues at somewhat disproportionate length where circumstances appeared to warrant it - Vietnam because of its long isolation, Australia because of its long alliance relationship with the United States, and South China Sea territorial claims because of the complexity of the issues.

For many years, the Center for Naval Analyses (CNA) has been charged with reconstructing and analyzing the Navy's major fleet exercises. Before deploying, a carrier battle group completes its work-up cycle by participating in a major exercise war. CNA provides reconstruction and analysis support at the request of the fleet commanders, who need an objective evaluation of their battle group's tactics and readiness. On 26 and 27 September 1995, Commander, Naval Base San Diego and Fleet Industrial Supply Center, Sand Diego hosted the 1995 San Diego Preparedness for Response Exercise Program (PREP) Area Exercise. This report documents our evaluation methodology, and offers a suggested evaluation plan for use in future PREP area exercises.

The Navy's Graduate Medical Education (GME) program consists of post-medical school programs to train physicians. These programs have been successful in improving the quality and dependability of sources of medical corps personnel. In recent years, the Navy has downsized GME somewhat. Further reductions in GME may be mandated because of budget constraints, but Medical Operational Support Requirements - which support the Marine Corps, ships afloat, and Navy bases outside the continental United States - may require some specialty programs to enlarge. Therefore, the Surgeon General of the Navy, via the Assistant Chief for Plans, Analysis, and Evaluation, asked CNA to study past medical corps attrition and build a model to assist with the planning of GME program size. This research memorandum addresses how the Navy can set priorities among its GME programs, a subject currently under study by staffs working for the Chief, Medical Corps, the Assistant Chief for Personnel Management, the Assistant Chief for Plans, Analysis, and Evaluation, and the former Health Services Education and Training command. This memorandum is intended to assist the work of these staffs.
